Delta Woods Middle School

Every student athlete who wishes to participate must have a current MSHSAA (Missouri State High School Activities Association) medical eligibility form on file and important medical and emergency information uploaded to the Privit website. 

**All medical eligibility forms are valid for 2 years, unless noted differently by the doctor.  Athletes must be in full participation to participate in practice/tryouts.**
