Sports Physicals
All students who plan to participate in MSHSAA-sanctioned activities must have a valid physical form on file before the first day of practice. Without it, students will not be allowed to try out or participate.
How to Submit the Physical Form:
During enrollment in PowerSchool:
1. Select “Student will participate in activities.”
2. When prompted in Section 5A, upload the completed MSHSAA Physical Form.
Tip: Make sure the form is signed and dated by a physician and includes all required sections.
